Resourceful Communities Partnership: Integrating place based health and wellbeing in rural Wales

Place based approaches to health and wellbeing require careful consideration of the way in which we think about the relationship between community led action and services, and how we work together to achieve integration. The session will examine through presentation and discussion the evidence for integration and how a cross sectoral group on the St Davids Peninsula is collaborating to coproduce a practical and workable social model across three wards.
